Forms and Links
All forms must by typed and submitted directly to the department’s Program Assistant. The department will scan the completed form, save it in your file and email it directly to College of Sciences and College of Graduate Studies. If a link is not working please contact the department by email at physics@ucf.edu.
- Department Candidacy Status Form Planetary Track - This form is for Planetary Track students and must be completed after the student has passed the Written exam (Research Paper) and completed the Oral Presentation. Students must also complete the Doctoral Committee/Candidacy Status form to obtain CANDIDACY STATUS
- Doctoral Committee/Candidacy Status Planetary Track Form - This form is for Planetary track students and must be submitted prior to passing the written exam in order to create a Dissertation committee and to notify Grad Studies that Candidacy Status has been obtained.
- Department Dissertation Proposal Planetary Track Form- This form is for Planetary Track students only to submit their Thesis/Dissertation proposal. It must be completed at least three semesters prior to defending and graduating.
- Department Dissertation Proposal and Candidacy Status Form Physics - This form must be completed after the student has passed the Written exam, completed the Dissertation Proposal and 51 credit hours. Students must also complete the Doctoral Committee/Candidacy Status form.
- Doctoral Committee/Candidacy Status Physics Form - This form is for Physics PhD students and must be submitted at least twice. Once immediately after passing the written exam in order to create a Dissertation committee. A second time once the student has completed 51 credit hours, passed the written exam and submitted a Dissertation Proposal. Students must submit this form along with the Department Dissertation Proposal and Candidacy Status Form to obtain CANDIDACY STATUS.
- GTA – Performance Assessment Form - This form is completed by the faculty supervisor of each GTA two weeks prior to the end of the semester. Failure to submit this form will result in a hold on a student’s account and possible cancellation of GTA contract.
- Graduate Certificate Completion Form - Students must complete this form to initiate graduate certificate completion.
- Graduate Petition Form - This form is used when a students is requesting an exception to university policy.
- Graduate Special Leave of Absence Form - A Special Leave of Absence may be granted to a student in order to temporarily waive the continuous attendance requirement.
- Late Fee Waiver Request Form - This form is used when a student is requesting a waiver of late payment fee, late registration fee, reinstatement fees or dropped for non-payment fee.
- Master’s Degree Certification Checklist - This form is completed when a student has completed all requirements in the Physics M.S. program and wants to be granted a M.S. degree.
- Master’s Exit Interview - This form is completed when a student has completed all requirements in the Physics M.S. program and wants to be granted a M.S. degree.
- Nomination and Appointment to Graduate Faculty and Graduate Faculty Scholars - This form is used by the department to approve a committee member that is not a faculty member at UCF.
- Ph.D Exit Interview - This form is completed when a student has completed all requirements in the Physics PhD. program and wants to be granted a PhD. degree.

A Program of Study for students seeking a PhD. degree should be on file with the College of Graduate Studies by the end of the third semester (based on full-time enrollment).
- Program of Study MS (When opening file, please make sure to enable content)
A Program of Study for students seeking an M.S. degree should be on file with the College of Graduate Studies by the end of the first semester (based on full-time enrollment).
- Purchase Request Form (Password Protected) - Please fill out all yellow highlighted cells and submit to Jessica Brooks or Monika Crittenden. PCard holders fill in light blue cells as well.
- Restricted Registration Agreement Form - This form is used to register for graduate courses such as Directed Research, Dissertation Hours and Directed Independent Studies.
- Thesis and Advisory Committee Form (MS Students Only) – This form is for MS students that have chosen the Thesis option. It must be completed prior to completing the departmental Thesis and Dissertation Proposal Form and the Thesis and Dissertation Attachment form and Thesis and Dissertation Approval Form.
- Thesis Proposal Form (MS Students Only) - This departmental form is used by MS students to submit their Thesis/Dissertation proposal. It must be completed by MS students after submitting a Thesis and Advisory Committee Form.
- Thesis and Dissertation Services - This is a link that will take you to UCF’s Thesis and Dissertation Services. You must follow the instructions on the website. This should be done the first week of the semester you intend to defend.
- Time Conflict Approval Form - This form is used so that a student could take two courses despite the time conflict.
- Transfer Request Form - This form is used for graduate students that are requesting credits to be transferred from previous institutions.
- Travel Approval Form (Password Protected) Please follow instructions on top of form and submit to Jessica Brooks orMonika Crittenden prior to traveling.
- Traveling Scholar Form - Required for students who wish to attend another institution as a Traveling Scholar.
